29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. At this workplace, employees conducted fabrication operations and operated the powered industrial truck without successful completion of training and the evaluation of the operator's performance. South Texas Steel Service Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(2)(ii), which was contained in OSHA inspection number 393082, citation number 2, item number 1 and was affirmed as a final order on 6/14/12, with respect to a workplace located at 226 E.
