29 CFR 1910.147(c)(6)(ii) The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. Employees were exposed to electrical shock, and caught-in hazards. An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR 1910.303(g)(2)(i)(A): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or by location in a room, vault, or similar enclosure that is accessible only to qualified persons: Employees were exposed to an electrical hazard. An electrical panel box rated at 460 volts was observed ajar and absent door latch or fasteners on the ARPAC Bundler #2, as known to company.
