29 CFR 1910.303(b)(1)(viii): Electrical equipment was not free from recognized hazards based on other factors that contribute to the practical safeguarding of persons using or likely to come in contact with the equipment: Near the kitchen walk-in cooler, on the wall where a used shelf sat, an unused but live 120 V duplex electrical receptacle/outlet had a broken blade exposed in the ungrounded slot ("hot") of the receptacle, therefore exposing anyone to 120 V electricity when coming in contact with the damage, such as when reaching into the shelf.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a. Above the drive-through counter, there was damage to a relocatable power tap (power strip) cord insulation at the plug end. b. In the kitchen, insulation at the plug end of a 240 V CollectralMatic fryer power cable was damaged. c. There was damage to the power cord entering the motor, on the fan used for cooling in the kitchen. Abatement documentation is required for this violation.
