29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: a) Jobsite - On or about January 24, 2016, a forklift operator moved and repositioned a GEHL RS6-34 Rough Terrain Forklift while an employee was working from the approved metal work platform that was not secured to the forks and carriage, resulting in struck-by and fall hazards of up to 15 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite - On or about January 24, 2017, an employee worked and stood on the mid-rail of an approved work platform with the access gate open while the platform was elevated and supported by a GEHL RS6-34 Rough Terrain Tele-handler forklift, resulting in a fall hazard of up to 15 feet.
