OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between hazards while performing change-out of a finished product: (a) On or about October 8, 2016, in the Extrusion Film Department, the employer did not ensure that employees were clear of the below-the-hook lifting device on the Harrington 3 Ton overhead underhung hoist when attaching the J hook onto the core of the Unwind Machine on the Davis Standard/Black Clawson extrusion EX06.
