29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:     a) Global Emergency Vehicles - On or about January 26, 2017, employees working in the Remount Shop were exposed to struck-by hazards from a Skil 6" bench grinder, that did not have the work rest adjusted to within 1/8 inch of the abrasive grinding wheel (left wheel).        Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    a) Global Emergency Vehicles - On or about January 26, 2017, oxygen cylinders were being stored in direct contact with an acetylene fuel gas cylinder, exposing employees to burn hazards from fire and explosion.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:    a) Global Emergency Vehicles - On or about January 26, 2017, oxygen cylinders chained against the wall in the Refurb Shop did not have the valve protection caps in place, exposing employees to compressed gas hazards, as well as burn hazards from fire and explosion.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:    a)  Global Emergency Vehicles - On or about January 26, 2017, employees perform tasks with flammable aerosols and perform oxy-acetylene torch cutting, where employees were provided fire extinguishers to use during incipient stage fire fighting, and the employer did not provide an educational program as required.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.    ABATEMENT NOTE: By this date the employer must either correct the alleged violation or implement a Fire Safety Policy, as outlined in 29 CFR 1910.38(a) and (b) which included the evacuation requirement of 29 CFR 1910.157(b).    Global Emergency Vehicles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(g)(1), which was contained in OSHA inspection number 1127938, citation number 1, item number 4, and was affirmed as a final order on April 29, 2016, with respect to a workplace located at 1991Hartell Ave. Levittown, PA 19057.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a)  Global Emergency Vehicles - On or about January 26, 2017, employees perform tasks with hazardous aerosol chemicals, such as but not limited to, Enamel Paints and Brake Parts Cleaner, and the employer did not provided effective information and training on hazards of such chemicals as required.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.      ABATEMENT NOTE:  Employees shall be informed of the following:  1. The requirements of this section;  2. Any operations where hazardous chemicals are present;  3. The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals and Safety Data Sheets.      Employee training shall include at least:  1. Methods and observations that may be used to detect the presence or release of hazardous chemical in the work area.  2. The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area.  3. The measures employees can take to protect themselves such as specific procedures and personal, protective equipment to be used.  4. The details of the employer's Hazard Communication Program including an explanation of the labels received on shipped containers and the workplace labeling system used by the employer; Safety Data Sheets, including the order of information and how employees can obtain and use the appropriate hazard information.     Global Emergency Vehicles was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(h)(1), which was contained in OSHA inspection number 1127938, citation number 1, item number 6-C, and was affirmed as a final order on April 29, 2016, with respect to a workplace located at 1991Hartell Ave. Levittown, PA 19057.
