29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a)Construction site at corner of Bibb Street and Catoma Street, Montgomery, AL: Employer failed to ensure employees using a 24 foot extension ladder to access a roof had the ladder extended at least 3 feet over the upper landing surface to prevent falls of 12 feet to a dirt/grass surface while performing roof shingling removal job tasks on a flat roof.
