Employees were not protected with hardhats while the crane was lifting a basket of materials overhead. Employees exposed to being struck by an object weighing approximately 1900 pounds.

Employees were lifting a bucket with a 4 leg wire rope sling that did not have any identification markings.

29 CFR 1926.251(a)(6): Each day before being used, and during use, where service conditions warrant, the sling and all fastenings and attachments were not inspected for damage or defects by a competent person, and damaged or defective slings were not immediately removed from service:  a) For employees working at 8371 Grand Palm Drive in Estero, as observed on or about January 24, 2017. Employees were using a crane with slings to lift the basket of material from the roof to the dumpster. The slings were inspected before the days use and were not removed from service when damage was identified.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) For employees working at 8371 Grand Palm Drive in Estero, as observed on or about January 24, 2017. Employees were working on the roof of a residential home without the use of a fall protection system. Employees were exposed to a fall of approximately 18 feet 8 inches.      Colonial Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1009546, citation number 01, item number 001 and was affirmed as a final order on December 10, 2014, with respect to a workplace located at 6201 7th Avenue West, Bradenton, FL 34209.
