29 CFR 1910.184(i)(1): Synthetic web slings:  Sling Identification.  Each sling shall be marked or coded to show the rated capacities for each type of hitch and type of synthetic web material:  On our about  January 21, 2017, employees were exposed to the hazard of being crushed by loads due to the fact that employees were using synthetic web slings that were not marked with legible identification markings, to include their rated capacities.

29 CFR 1910.303(b)(2):  Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:  On or about January 27, 2017, employees were exposed to electric shock and fire hazards when using a Relocatable Power Tap (outlet strip) to power two block heaters on equipment in the yard.  The Relocatalble Power Tap (RPT) was daisy chained to an extension cord that supplied power to the RPT from a receptacle inside the "old machine shop".  The RPT was permanently secured to a post out in the yard.     Underwriters Laboratories Inc. listing requirements for RPTs state the following: "RPTs are intended to be directly connected to a permanently installed branch circuit receptacle.  RPTs are not intended to be series connected (daisy chained) to other relocatable power taps or to extension cords.  RPTs are not intended to be permanently secured to building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring.

29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used to run through holes in walls, ceilings, or floors.  On or about January 27, 2017, employees were exposed to electric shock and fire hazards due to the fact that extension (flexible) cords were routed through a hole in the wall of the "old machine shop" to power block heaters on equipment in the yard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about January 27, 2017, employees were exposed to electrical shock hazards due to the fact that a flexible cord, with inadequate strain relief, was being used to power a block heater on a piece of equipment in the yard.

29 CFR 1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  On or about January 17, 2017, employees working in the washbay and old machine shop, were exposed to hazardous chemicals, including oil, antifreeze, and Ag Master hydraulic tractor fluid and the containers were not marked with general information regarding the hazards of the chemicals.
