Occurred on or about 1/25/17.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Farmall Lane, Building 2600, Monroe Township, NJ:  Employees were using an aluminum extension ladder to access the third floor through a window opening and although a grasping device was present, the ladder was not secured in place.  This hazard was observed on 1/25/17.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) Farmall Lane, Building 2600, Monroe Township, NJ:   The aluminum extension ladder which was being used to access the third floor had damage, such as but not limited to a broken locked mechanism and broken foot, and had not been removed from service.  This hazard was observed on 1/25/17.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Farmall Lane, Building 2600, Monroe Township, NJ:  Employees engaged in residential framing activities while working from a steep pitched roof were exposed to fall hazards of up to 40 feet to the ground below and were not protected by an adequate fall protection system.  This hazard was observed on 1/25/17.  GKL General Contractor Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1171135, citation number 1, item number 1a, and was affirmed as a final order on 12/2/2016, with respect to a workplace located at Lot 29 Barkshire Boulevard, Middletown, DE.
