29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: For the employees operating the bailer by the feed mill panel on or about January 23, 2017 and times prior there to, the employer did not install a guard to protect employees from the point of operation of the hydraulic compactor ram. This exposes employees to crushing and caught in between hazards.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected. For the employees operating the pneumatic railcar gate opener on or about January 23, 2017 and times prior there to, the employer used water clamps to secure end connectors and splice the air hose with working air pressure of 100 psig. This exposes employees to the hazard of air embolism, or cuts and abrasions from a failed clamp connection.
