29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about; January 30, 2017 Location: Crestwood Lake Apartments in Yonkers, NY, back of the building 72 at Beaumont Circle a) Five employees digging a trench by hand approx. 30 inches deep with spoil material approx. 4 ft height containing rocks and loose soil within 1 ft from the edge of the excavation were not protected from struck by hazards while not wearing hard hats .

Protection was not provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: On or about; January 30, 2017 Location: Crestwood Lake Apartments in Yonkers, NY, back of the building 72 at Beaumont Circle a) Five employees digging a trench by hand approx. 30 inches deep were not protected from struck by hazards from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations.
