29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  For the employees operating the Milwaukee chop saw on or about January 24, 2017 and times prior there to,  The employer did not ensure that the guard on the chop saw effectively guarded the fiber disk blade.  The adjustable portion of the guard had been wedged above the top of the blade guard and was no long capable of being moved to guard the blade.  This exposes employees to the hazard of being struck by flying debris and contact with the unused portion of the blade.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  For the employees operating the Baldor bench grinder to smooth welds and to de-burr metal parts to be installed on the concrete mixing trucks on or about January 24, 2017 and times prior there to, the employer did not ensure the work bench was properly adjusted to 1/8" from the wheel.  This exposes employee to the potential hazard of amputation by the abrasive wheel pulling a body part into the machine.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  For the employees operating the Baldor bench grinder to smooth welds and to de-burr metal parts to be installed on the concrete mixing trucks on or about January 24, 2017 and times prior there to, the employer did not ensure the tongue guard was adjusted to within 1/2 in of the stone.  This exposes employees to the hazard of being struck by debris during grinding operations.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  For the employees working in the mechanics truck shop on or about January 24, 2017 and times prior there to, the employer did not ensure employees exposed to splashing of acid contained in the lead acid batteries had immediate access to the emergency eye wash.  The eyewash was blocked by a cutting torch and other shop equipment. The eyewash bowl had material such as, but not limited to a container of hand cleaner that also blocked access to the eyewash.  The eyewash had not been tested regularly to assure proper function.  This exposes employees to the hazard of chemical burns due to delays in accessing the emergency eye wash.
