29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  For the employees performing roofing activities on or about January 25, 2017 and times prior there to;  The employer did not ensure employees were protected from falls of approximately 8 feet while installing roof sheeting material on a 12 pitch roof.  This exposes employees to the hazard of falling from heights of greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  For the employees accessing the roof of the new construction home on or about January 25, 2017, and times prior there to;  The employer did not provide a ladder capable of extending the side rails 3 feet above the roof.  This exposes the employees to falls when transitioning from the ladder to the roof and from the roof to the ladder.
