29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Waterbury Location: The employer did not ensure that the Langston Die Cutter machine was equipped with an adequate guard that would prevent an employees hand from entering the machines rollers. On or about 01/30/2017, an employee sustained severe injuries which resulted in an amputation of four fingers of his dominant hand after his hand was pulled into a die cutter machine's rollers.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees. Waterbury Location: An employee, as "affected and/or other type" was not provided with training and information in the controlling of potentially hazardous and unexpected releases of energy sources (lockout/tagout) during servicing, maintenance and unjamming of equipment, such as (but not limited to) the Langston Die Cutter machine. On or about 01/30/2017, an employee sustained severe injuries which resulted in an amputation of four fingers of his dominant hand after his hand was pulled into a die cutter machine's rollers while attempting to clear a cardboard jam.
