29 CFR 1910.134(d)(3)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace:    a)  On site at Sheetz Distribution Services, 242 Sheetz Way, Claysburg, PA, on or about January 22, 2017 - Industrial Caulk & Seal, Inc. did not identify and evaluate the conditions at the worksite with an accurate representation of employee exposures.  Employees were operating a gas and a propane powered generator indoors during work activities.    On or about January 22, 2017, an employee designated as a Sheetz Team Lead was exposed to an airborne concentration of carbon monoxide (CO) in excess of 50 ppm as an 8-hour time weighted average (TWA) exposure.  The employee was exposed to carbon monoxide during an approximately 325 minute occupational exposure at a mean calculated 272.2 parts per million (ppm) carbon monoxide level.  The percent carboxyhemoglobin (COHb) in the employee's blood reached a calculated peak level of 21.0%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 184.3 ppm CO which is 3.69 times the OSHA Permissible Exposure Limit (PEL) of 50 ppm CO.      On or about January 22, 2017, an employee designated as a Sheetz Team Member was exposed to an airborne concentration of carbon monoxide (CO) in excess of 50 ppm as an 8-hour time weighted average (TWA) exposure.  The employee was exposed to carbon monoxide during an approximately 360 minute occupational exposure at a mean calculated 105.8 parts per million (ppm) carbon monoxide level.  The percent carboxyhemoglobin (COHb) in the employee's blood reached a calculated peak level of 10.0%.  Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account.  The 8-hour TWA exposure was 79.3 ppm CO which is 1.59 times the OSHA Permissible Exposure Limit (PEL) of 50 ppm CO.
