29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the sequence of actions outlined in 1910.147(f)(1)(i) thru 1910.147(f)(1)(v) shall be followed: On or about January 27, 2017, at or near 13111 Hwy 190 W in Hammond, LA; employees failed to follow the sequence of actions outlined in 29 CFR 1910.147(f)(1)(i) thru (f)(1)(v) when lockout and/or tagout device(s) had to be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment, or component thereof. Specifically, during troubleshooting, employees were not removed from the machine or equipment area before remaining actions in the sequence were followed.
