29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. For the employees installing roofing sheet metal screws on the flat roof of the new construction self storage facility on or January 25, 2017 and times prior thereto; The employer did not ensure employees were provided fall protection when working at heights of approximately 8 feet above the ground. This exposed employees to the hazard of falling from heights of greater than 6 feet.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigging. For the employees working on the metal roof on or about January 25, 2017 and times prior thereto; The employer did not provide a ladder suitable for accessing the roof of the self storage facility. This exposed employees to the hazard of falling during the transition from the ladder to the roof and from the roof to the ladder.
