29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about January 27, 2017, at Lot 11, 5735 Read Road, Sugar Hill, Georgia:  Two employees performing framing work were exposed to struck-by hazards from falling tools and materials from employees working above them from the top plate as the employees worked from the ground without wearing head protection.

On or about January 27, 2017, at Lot 11, 5735 Read Road, Sugar Hill, Georgia:  Employees performing framing work were exposed to struck-by hazards from wood chips, nails, and saw dust while operating nail guns and circular saws without wearing eye protection.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.  On or about January 27, 2017, at Lot 11, 5735 Read Road, Sugar Hill, Georgia:  Two employees performing framing work were exposed to electric shock hazards while operating Makita Model 5007F, 120-volt, 15-amp, circular saws receiving power from a damaged flexible cord when the employer did not inspect and remove the damaged equipment and cord from service.
