29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  (a) 82 Longview Drive, Mifflin Pennsylvania - The employer did not ensure that the entrance area to the stairway opening was provided with a mid-rail to prevent employees from falling to the lower level, on or about January 30 2017.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103)  (a)  245 Swamp Road, Mexico Pennsylvania  - Employees wear dust mask when sanding dry wall.   The employer did not provided Appendix D of the respiratory standard to employees when they voluntarily wore dust masks, on or about January 31, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not develop, implement and maintain a written hazard communication program which describes how the criteria of labels and other forms of warning, safety data sheets, and employee information and training will be met, on or about January 31, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
