29 CFR 1926.451(e)(1): Scaffold platforms that were more than two feet (0.6m) above or below a point of access, were not provided with a means of access:  On or about August 29, 2016, at a shopping center renovation site located at 10122 E. Memorial Road in Tulsa, Oklahoma, employees were observed working on a scaffold that did not have a ladder or other means of safe access. Employees were exposed to a fall hazard.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway:  On or about August  29, 2016, at a shopping center renovation site located at 10122 E. Memorial Road, Tulsa, Oklahoma, employees were observed walking across a wood plank used as a walkway to access different sections of scaffold. The walkway did not have guardrails erected, exposing employees to the hazard of falls.
