Section (5)(a)(1) of the Occupational Safety and Health Act of 1970: The employer does not furnish to each of his employees employment and a place of employment which were free from the recognized hazard of being struck-by which causing or likely to cause death or serious physical harm to employees.   On or about January 31, 2017, at a well drilling location where an employee was exposed to struck by hazards while working on hoist cables without the line being securely fastened.

29 CFR 1910.28(b)(9)(ii)(A): The employer did not provide a personal fall arrest system or a ladder safety system throughout the entire vertical distance of the ladder, including all ladder sections:  On or about January 31, 2017, at a well drilling location where an employee was exposed to a fall hazard while working from the fixed ladder of a work over rig.
