29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On 02/01/2017, Lippmann Printing & Graphics LLC, located at 2385 Grissom Drive, St. Louis, MO, did not have an Energy Control Program. The employer had equipment with more than one energy source which would require such a program. The following equipment required an Energy Control Program: A Challenge Paper Cutter, Model: MC, S/N: 12214, with 230 volt electrics, 1000 psi hydraulics, and potential (gravity) energy; and A Graphics Whizard, Model: CM PLAT, S/N: 2609104, with 120 volt electrics and 106 psi pneumatics (air) pressure.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On 02/01/2017, Lippmann Printing & Graphics LLC located at 2385 Grissom Drive, St. Louis, MO, had not ensured listed or labeled equipment was used or installed in accordance with any instructions included in the listing or labeling. In the Digital Print Room, there was a drop pendant which incorporated a metallic 4 receptacle electrical box with knock outs/punch outs. This electrical box was not designed to be used as a drop pendant box.
