29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a- 1068 Route 206 South, Bordentown NJ: On or about 01/30/17, company workers performing roof preparation and other related activities were exposed to fall hazards in excess of 18 feet to the ground surface below when working/being lifted to & from the building's roof while on a wooden platform that was resting on the forks of a JLG All-terrain Forktruck.  This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:    a- 1068 Route 206 South, Bordentown NJ: Employees were exposed to fall hazards when they attached their personal fall arrest system's lanyard to the All-terrain forktruck's backrest eyebolt.  This eyebolt was designed and installed to secure the attachment to the All-terrain fork truck and was not rated to support 5,000 pounds per employee attached. Condition noted on or about 01/30/17. 29 CFR 1926.503(a)(1): A training program was not provided to each employee who might be exposed to fall hazards, to enable that employee to recognize the hazards of falling, and be trained in the procedures to be followed to minimize these hazards:    a- 50 Sidney Avenue, Deal NJ: On or about 01/30/17, company workers performing roof installation, measuring and other related activities were exposed to fall hazards when improperly utilizing fall protection when riding and working from/off of a unsecured, wooden pallet that was sitting on the forks of an all-terrain fork truck.  These employees had not received fall hazard recognition and minimization training from the company.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
