29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Carpet Staging Area: On or about January 31, 2017, and at times prior, the employer exposed employees to struck-by hazards in that the employer did not obtain approval from the forklift manufacturer prior to using the forklift squeeze attachment while employees were using the forklift to transport bales of carpet.
