(b) On or about February 1, 2017 and at times prior to that date, for the employees exposed to caught-in and/or amputation hazards while operating a Hardinge lathe, Serial No. HSL-5C-5210T in the slotting room located at 417 Main Avenue West, Rolla North Dakota.      (c) On or about January 30, 2017 and at times prior to that date, for the employees exposed to caught-in and/or amputation hazards while operating a Hardinge lathe, Model HLV-H in machine shop one located at 417 Main Avenue West, Rolla North Dakota.      (d) On or about January 30, 2017 and at times prior to that date, for the employees exposed to caught-in and/or amputation hazards while operating a Bridgeport vertical milling machine, Serial No. J250847 in machine shop one located at 417 Main Avenue West, Rolla North Dakota.
