29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: (a) Reynolds Square Townhomes, Rear side of Unit #7: On or about January 31, 2017, and at times prior, the employer exposed employees to electrical shock/burn hazards while using a job made outlet panel, in that the electrical outlets were not properly supported, installed and fixed in accordance with the NEC. GFCI outlet covers remained in the open position which exposed live parts to the weather.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: (a) Reynolds Square Townhomes, Unit #7: On or about 01/31/017, and at times prior, the employer exposed employees to fall hazards of approximately 7-feet to the lower concrete surface, in that stair rails were not installed along the open side of the stairway as employees used this stairway to access the upper levels of the structure to perform framing activities.
