29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices shall be singularly identified; shall be the only devices(s) used for controlling energy; shall not be used for other purposes; and shall meet the following requirements: a. #2 Panel, Heater: On or about January 27, 2017, employees were exposed to electrical, caught-in, and amputation hazards, in that a lockout device used to control hazardous energy for the heater was not singularly identified with the employee who applied the lock. b. #1 Panel, West Wall: On or about January 27, 2017, employees were exposed to potential unintended machinery startup, in that lockout devices (8) used to control hazardous energy were not singularly identified with the employee(s) placing the lock.
