29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the workplace: Security locations at the Social Security Administration District Office in Augusta, ME, the Muskie Federal Office Building in Augusta, ME, and the Smith Federal Office Building in Bangor, ME - The employer did not select the appropriate body armor/protective vests in that the provided armor/vests did not meet National Institute of Justice (NIJ) Level IIA to protect employees from hazards of bullets and other similar projectiles as required per security contract and NIJ Standard 0101.06.

29 CFR 1910.132(f)(1)(iii): Employees required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: Security locations at Social Security Administration District Office in Augusta, ME, the Muskie Federal Office Building in Augusta, ME, and the Smith Federal Office Building in Bangor, ME - a) Employees searching purses, briefcases, bags, etc. were not provided effective training and did not know how to doff protective gloves which would prevent employees from contact with a biological substance or contact with toxic, caustic, or noxious substances. b) Employees wearing vests improperly adjusted to protect their sides, chests and torsos were not trained to properly adjust and wear body armor/protective vests to ensure protection from bullets and other projectiles
