29 CFR 1904.32(a)(1): At the end of each calendar year the employer did not review its OSHA 300 log to verify that the entries were complete, accurate, and correct any deficiencies identified: a) At the facility, where the 2016 OSHA 300 log was not complete and accurate. An injury that occurred on April 29, 2016, where an employee received a flash burn and a corneal abrasion in the left eye welding on a pipe, was not recorded. c) At the facility, where the 2016 OSHA 300 log was not complete and accurate. An injury that occurred on April 7, 2016, where an employee fractured/chipped his front tooth after being hit in the mouth with a carabineer was not recorded. d) At the facility, where the 2016 OSHA 300 log was not complete and accurate. An injury that occurred on March 12, 2016, where an employee tore his left bicep while trying to catch a falling pipe was not recorded. e) At the facility, where the 2016 OSHA 300 log was not complete and accurate. The employer failed to describe the object or substance that directly injured the employees in column (F) for case numbers 1 through 8.
