29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are not kept in a clean, orderly, and sanitary condition.  On or about 01/31/2017, at Skywalker Communications, located at 1760 West Terra Lane, St Louis, MO  63366, employees were exposed to tripping hazards in the maintenance area across from isle 22 created by items such as, but not limited to, sawdust, loose boards, electrical cords, boxes, old equipment and other debris.

29 CFR 1910.110(f)(2)(i): Containers in storage were not located to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons:  On or about 01/31/2017, at Skywalker Communications, located at 1760 West Terra  Lane, O'Fallon, MO 63366, employees were exposed to struck by hazards where propane tanks used for fueling forklifts were not properly secured.  Propane tanks were being stored next to east wall at isle 20 in a high traffic area.

29 CFR 1910.178(p)(1): A powered industrial truck is found to be in need of repair, defective, or in any way unsafe, was not taken out of service until restored to a safe operating condition:   1/  On or about 01/31/2017, at Skywalker Communications, Inc., located at 1760 West Terra Lane, O'Fallon, MO  63366, Doosan Forklift, unavailable Model/Serial#s, was being used by employees , with an inoperable back-up alarm.  2/  On or about 01/31/2017, at Skywalker Communications, Inc., located at 1760 West Terra Lane, O'Fallon, MO  63366, Clark Forklift, unavailable Model/Serial#s, was being used by employees, with an inoperable back-up alarm and horn.  3/  On or about 01/31/2017, at Skywalker Communications, Inc., located at 1760 West Terra Lane, O'Fallon, MO  63366, Yale Forklift, Model # ERPS30TFN36SE082, Serial #A807N03205S, was being used by employees,  with an inoperable back-up alarm.
