29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about January 31, 2017; at the construction site: An employee was stepping out from the rooftop onto a ladder that was propped up against the wall a few feet from the roof. The ladder did not extend at least 3 feet above the rooftop. Employee was exposed to a fall of at least 14 feet.
