(a)  Right side of the building (roof) - On or January 26, 2017 an employee inside an aerial lift was exposed to a flying object hazard while using a nail gun without wearing eye protection.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    (a)  Front of the building (right side of the entrance) - On or about March 3, 2017 two employees were exposed to a fall hazard while working on a scaffold 14 feet above concrete with a ladder that did reach the top platform. The ladder did not reach the 2nd level of the scaffold.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, shall not be used on top of scaffold platforms to increase the working level height of employees.    (a)  Front of the building (right side of the entrance) - On or about March 3, 2017 an employee was exposed to a fall hazard due to a box or device being placed on the scaffold platform to increase the working height. The scaffolds platform height was 14 feet above concrete.

Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a)  Right side of the building (roof) - On or January 26, 2017 two employees were exposed to a fall hazard while working on a roof with no fall protection 26 feet above concrete.
