29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about January 30, 2017, employees roofing installation on a flat roof were exposed to struck by hazards while employees worked on the ground when the employer failed to mandate the use of hard hats.    No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   On or about January 30, 2017, employees performing roofing installation at a commercial property were exposed to fall hazards of 11' while working from the second level when the employer failed to ensure fall protection was utilized.  No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about January 30, 2017, employees performing roofing installation on the flat roof of a commercial property were exposed to 11' fall hazards and the employer failed to ensure employees were adequately trained to recognize and minimize the hazards associated with a fall.  Abatement certification is required of this item in accordance with requirements of 29 CFR 1903.19(c).
