29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)    (a) Facility Wide - On February 1, 2017, the employer failed to ensure that all powered industrial trucks that were found to be in need of repair or were in anyway unsafe were removed from service until they had been restored to a safe operating condition. The horn on forklift #2 and #8 were not functioning.    No abatement certification or documentation is required for this item.        U.S Postal Service was previously cited for a violation of this Occupational Safety and Health standard, 29 CFR 1910.178(p)(1) which was contained in OSHA Inspection Number 1201606, citation number 1, Item number 4 and was affirmed as a final order on March 29, 2017 with respect to the workplace located at 11251 Rancho Carmel Drive, San Diego, CA 92199.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed:    (a) Belt 344 - On February 1, 2017, the employer did not ensure that exit routes were kept free and unobstructed.  BMC and APC carts were staged in the area reducing the exit access to 20 inches.      In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.      U.S Postal Service was previously cited for a violation of this Occupational Safety and Health standard, 29 CFR 1910.37(a)(3) which was contained in OSHA Inspection Number 1186652, citation number 1, Item number 1 and was affirmed as a final order on December 13, 2016 with respect to the workplace located at 28201 Franklin Pkwy, Santa Clarita, CA 91383.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  (a) South Dock at Dock #14 - On February 1, 2017, the employer did not ensure that portable cord and plug connected equipment was visually inspected before use. The plug used to power a dock light was damaged and detached from the cord.   No abatement certification or documentation is required for this item.
