29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Production area;  On or about 2/2/17, the employer did not assess the hazards present during various tasks and what personal protective equipment is necessary to protect employees.  Employees work at and on injection molding machines without any eye/face protection.  Mold release and cleaners, containing solvents such as toluene and trichloroethylene are used without any hand protection.

Two instances where eye hazards existed were: 1. When the injection mold tools were opened for any reason during operation or maintenance 2. When the molds were cleaned using spray mold cleaner that contained solvents, such as toluene and trichloroethylene

29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde  Production area;  On or about2/2/17, employees working at or near injection molding tools using acetyl copolymer were not monitored for exposure to formaldehyde, which is a product of heating acetyl copolymer.  Sampling performed on 2/28/117 indicated that formaldehyde was produced during the process.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):    Production area;  On or about 2/2/17, the employer did not have a written hazard communication program.  Employees work at or near plastic injection molding machines that heat various plastics during the process, one of which, acetyl copolymer, gives off formaldehyde when heated.  Mold cleaners containing toluene and trichloroethylene are also used by employees.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Production area;  On or about 2/2/17, employees had not received hazard communication training for chemical hazards in their work area, including formaldehyde, which is produced when acetyl copolymer is heated.  Toluene and trichloroethylene are also present in the mold cleaners used.
