(b) On or about February 2, 2017 and at times prior to that data, for the employees exposed to struck-by and/or caught-in hazards while operating a HAAS CnC machine, model VF4, Serial No. 1084088 where the interlock door safety device had been removed and modified to simulate door closure at 2331 North 42nd Street, Grand Forks, North Dakota.      Abatement Note: Among other methods, one feasible and acceptable abatement method to correct the hazard would be to install a functioning door interlock safety device on the HAAS CNC machines in accordance with ANSI B11.22 Safety Requirements for Turning Centers Automatic Numerically Controlled Turning Machines, paragraphs 6.21 Safeguarding and 6.21.2 Automatic Mode which states:      1. Paragraph 6.21: Machine shall be safeguarded whenever practical by fixed or interlocked movable guards.      2. Paragraph 6.21.1.2: Machine operation shall only be possible when the guards are closed and the safeguarding (protective) devices are active. If it is possible, in this mode, to open an interlocked moveable guard, the hazardous movement shall cease and be inhibited.      If opening of the interlocked movable guard gives access to hazards, guard locking shall be provided.
