29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) High and Low Hazard Area - on or about February 2, 2017, employees performed maintenance on various pieces of production machinery to include, but not limited to, the ABL processing equipment and Reyco chop stand where lock out/tag out hardware was used, but an annual inspection review of the energy control procedures and between an inspector and each authorized employee of that employee's responsibilities under the energy control procedure was not conducted.  b) High Risk Area - on or about February 2, 2017, the employees performed sanitation on various machinery where lock out/tag out hardware was used, but a periodic inspection of the energy control procedures was not performed at least annually to ensure that the procedures and the requirements of the standard were being followed.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) High Hazard Area - on or about February 2, 2017, an employee used an automatic melon peeler for production work, but the employer had not provided adequate guarding of the point of operation to prevent employees from coming into contact with the moving blade.
