On or about February 1, 2017, at the above addressed jobsite, employees performing carpentry operations from rafters approximately nineteen (19') feet above the ground were not protected from falls, thereby exposing the employees to fall hazards.

On or about February 1, 2017, at the above addressed jobsite, employees conducting carpentry operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.
