29 CFR 1904.32(b)(6): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) for the previous year was not posted between February 1st and April 30th:  (a) Department of Veterans Affairs Medical Center 1500 Franklin Street, NE Washington, DC - On or about 02/02/2017, the employer did not post a 300A Form (annual summary) for calendar year 2016.  The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard 29 CFR 1904.32(b)(6), which was contained in OSHA inspection number 1034563, citation number 2,item number3 and was affirmed as a final order on 12/04/2015 , with respect to a workplace located at 1030 Jefferson Avenue, Memphis, TN 38104.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.29(b)(8): The employer classified other types of injuries and illnesses as privacy concern cases:  (a)  Department of Veterans Affairs Medical Center  - On or about February 2, 2017, the employer's 2016 300 injury and illness log classified case number  2017-00022, which involved an employee being splashed in the eye with blood, as a privacy case.  (b)  Department of Veterans Affairs Medical Center  - On or about February 2, 2017, the employer's 2016 300 injury and illness log classified case number 2017-00045 which involved an employee being splashed in the eye with blood, as a privacy case.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1904.32(b)(1): The employer did not extensively review the OSHA 300A Annual Summary to ensure that entries are correct and complete:  (a)  Department of Veterans Affairs Medical Center  - On or about February 2, 2017, the employer's 2016 300 log of injuries and illnesses, did not have a check in columns g through J  for case# 2017-00042 which led to the total number of injuries and illnesses on the 2016 300A annual summary being incorrect.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
