29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: H2S Scavenger Tower, M&B Compressor Station: On or about January 27, 2017, employees were exposed to a struck-by hazard due to an exploding iron vessel. An employee introduced Hydrogen Peroxide solution (Strong Oxidizer) into the vessel which reacted with the scavenger mixture and created an explosion. The employee was not trained nor aware of the potential chemical reaction.
