Employees were servicing and installing electric signs and other components while working from aerial lifts at heights, including Elliott HI Reach model ECL 3 55 CH, where employees were exposed to struck-by, caught-in, and fall hazards. 29 CFR 1926.454(b): The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the following topics, as applicable: 29 CFR 1926.454(b)(1) to 29 CFR 1926.454(b)(4).    Identity Services LLC, Geneva Commons job site: on or about February 3, 2017, employees involved in moving and operating the Elliott HI Reach model ECL 3 55 CH aerial lift were not trained by competent person to recognize crushed-by, caught-in, and struck-by hazards. Such employees were permitted to extend and maneuver the aerial lift's platform (basket) within the confined area of a bell tower's upper structure in close proximity to overhead hazards. Employees were exposed to unintended activation of the lift's upper operating controls likely to result in injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Identity Services LLC, Geneva Commons job site: on or about February 3, 2017, employee(s) operating an Elliott HI Reach model ECL 3 55 CH were exposed to injury as the employer failed to ensure safe working conditions of the aerial lift through daily control tests. Employee(s) were working from the aerial lift's platform (basket) at heights while operating the lift's upper controls with a bypassed redundant safety device (foot pedal), and illegible and mislabeled controls. Employees were exposed to hazards associated with unintended activation of lift's upper operating controls likely to result in death or serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.453(b)(2)(ix): Articulating boom and extensible boom platforms, primarily designed as personnel carriers, shall have both platform (upper) and lower controls. Upper controls shall be in or beside the platform within easy reach of the operator. Lower controls shall provide for overriding the upper controls. Controls shall be plainly marked as to their function. Lower level controls shall not be operated unless permission has been obtained from the employee in the lift, except in case of emergency:    Identity Services LLC, Geneva Commons job site: on or about February 3, 2017, employees working at height in the confined bell tower from the Elliott HI Reach model ECL 3 55 CH aerial lift's platform (basket) were exposed to injury as the platform leveling (tilt) toggle switch on the upper controls was incorrectly marked with its function reversed. Also, the surface of the plastic cover for the function label plate on the upper control box was damaged with cracks, chips, scratches and paint covering some areas making it illegible.     There is no abatement documentation or certification required for this item.
