29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) On or about 1/14/17, on the left side (when looking at the front of) a house at 41 Lexington Avenue, Buffalo, NY: Employees were standing on the 12 inch wide aluminum "pics" of a ladder jack scaffold while engaged in roofing work. The employees were exposed to a fall of approx. 18 ft. to the ground.  No fall protection system was utilized.      NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) On or about 1/14/17, on the rear dormer roof on the left side (when looking at the front) of a house at 41 Lexington Avenue, Buffalo, NY: Employees engaged in roofing work on a steep roof were exposed to a fall of approx. 20 ft.  No fall protection system was utilized. Pitch of the roof was approx. 14 in 12.     NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 1/14/17, on the left side (when looking at the front of) a house at 41 Lexington Avenue, Buffalo, NY: Employees used an extension ladder to access the 12 inch wide aluminum "pics" on the ladder jack scaffold that were approx. 17 ft. above the ground. The side rails of that ladder only extended approx. 12 inches to 24 inches above the "pic".       NO ABATEMENT CERTIFICATION REQUIRED
