29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: (a) Baker Concrete Construction, 222 Milwaukee Street, Denver, CO 80206: On and before February 1, 2017, the creating employer did not ensure that the scaffold system in use northwest of the tower crane was capable of supporting its own weight and at least 4 times the maximum intended load applied to it. The employer removed the structural support of the scaffold system. A general contractor employee accessed a scaffold platform to conduct carpentry work. The scaffold platform collapsed, and the general contractor employee fell 22.5 feet. This condition exposed construction workers in this area to a fall hazard of 22.5 feet.

An general contractor employee was performing carpentry work on the scaffold platform. The scaffold platform collapsed, and the general contractor employee fell 22.5 feet. This condition exposed construction workers in this area to a fall hazard of 22.5 feet.

Two employees removed the scaffold system structural support located northwest of the tower crane. A general contractor employee was performing carpentry work on this platform. The scaffold platform collapsed, and the general contractor employee fell 22.5 feet. This condition exposed construction workers in the area to a fall hazard of 22.5 feet.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. (a) Baker Concrete Construction, 222 Milwaukee Street, Denver, CO 80206: On or before February 1, 2017, the creating employer did not provide a safe ladder access to the scaffold platform located on the northwest side of the tower crane. The scaffold platform measured 28 inches lower than the deck used to gain access to the scaffold platform. A general contractor employee stepped down onto the scaffold platform. This condition exposed construction workers in this area to a fall hazard.
