29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: On or about January 23, 2017, employee were exposed to impalement hazards from the unprotected connection bolts when the cover on top of solvent tanks were removed to obtain depth measurements.

29 CFR 1910.140(c)(10)(i): The employer did not ensure snaphooks and carabiners were not connected directly to webbing, rope, or wire rope: On or about January 23, 2017, employees were exposed to fall hazards in excess of 10 feet while standing outside the guardrail system taking measurements on an alcohol solvent tank and the employer had the employee wrap his lanyard around the guardrail and attach his snaphook/carabiner directly to the lanyard.
