29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a- 315 Monmouth Avenue, Lakewood NJ: Employees, not supplied with fall protection, were exposed to fall hazards in excess of 26 feet to the ground surface below when performing EIFS preparation and other masonry related activities from the top buck of a prefabricated, tubular welded framed scaffold that was missing guardrails, was not fully planked, contained damaged and mixed components, was missing safe access/egress from the walking/working level, etc. In addition the worksite company employees had not received any scaffold hazard recognition and minimization training, that meets with the requirements of the standard, from the company.  Condition observed on or about 02/06/17.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

These openings had a vertical distance greater than 16 3/4 inches apart. Condition observed on or about 02/06/17.
