29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) Knepper Construction at 345 Vine Street, Denver, CO 80206: On or before February 6, 2017, the controlling employer did not ensure that masonry employees working near a window well opening were using appropriate fall protection. An employee was performing clean-up activities, and was not protected from falling via personal fall arrest systems, covers, or guardrail systems. Masonry subcontractor employees were performing brick-laying and were not protected from falling via guardrails, personal fall arrest systems, or a controlled access zone. This condition exposed employees to fall hazards of approximately 12 feet from ground to the bottom of the window well.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: (a) Knepper Construction at 345 Vine Street, Denver, CO 80206: On or before February 6, 2017, the controlling employer did not ensure that an extension cord was protected from damage. An extension cord crossed through the travelled area of a skid loader and was not protected from damage. This condition exposed employees to a 120 volt electrical hazard.
