29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section.      On or about 2/02/2017, Machine Technology, Inc. did not utilize energy control procedures while locking out their equipment during repairs. The company had procedures, but was not using them to facilitate repairs.  At the time of the inspection, an employee was repairing a Mazak Super Quick Turn machine, model SQT-15M-Y.  The machines air supply was not locked out and the procedure was not utilized.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     On or about 2/02/2017, employees from Machine Technology, Inc. were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  The following machines did not have guards:    Bridgeport Milling Machine, serial # 184313;  WEBB Horizontal Lathe, model WL 435, serial # 9-8106-18;  Bench top drill, no serial number;  16 speed drill press, model 793.12888851;  Enco Drill Press, model 126-2170.    Machine Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 191510, citation number 1, item number 1, issued on 3/12/2012, and was affirmed as a final order on 4/04/2012, with respect to a workplace located at 1721 Larkin Williams Road, Fenton, MO 63026.
