29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a.  On or about 01/31/2017, Caribbean Cinemas of St. Croix building - Scaffold roof area:  An employees was working over a two bed mobile scaffold in which its working platform area was not fully planked.  NOTE:  Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a. On or about 01/31/2017, Caribbean Cinemas of St. Croix building - Scaffold roof area:  No access ladders were provided to employees needing to access at a working level of 12ft., on two tier mobile scaffold.   NOTE:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:(a  a. On or about 01/31/2017, Caribbean Cinemas of St. Croix building - Scaffold roof area:   Employees were using a portable A type ladder which not extending at least 3 feet above the upper landing surface to which the ladder was used to gain access from the roof parapet to the mobile scaffold deck area.  NOTE:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.

29 CFR 1926.452(w)(1):   Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections shall be secured.  a.   On or about 01/31/2017, Caribbean Cinemas of St. Croix building - Scaffold roof area:  An employee was working over a two bed mobile scaffold in which four sets of cross braces were missing.  NOTE:  Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
