29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    a. On or about 2/02/2017, at  Sunshine Supermarket - main supermarket area. The employer did not perform a hazard analysis prior to start the supermarket clean-up operations in order to provide the necessary personal protective equipment. The clean-up operations which include debris removal, sweeping and collecting dust and garbage, exposure to bats.     NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:     a. On or about 2/02/2017, at  Sunshine Supermarket - main supermarket area. The employer did not provide the basic advisory information on respirators, as presented in Appendix D of this section, in written or oral format to employees who wore a half-mask (3M  Model 8210) dust respirator on a voluntarily basis during the clean-up operations which include debris removal, sweeping and collecting dust and garbage, exposure to bats feces.      NOTE:  Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

29 CFR 1910.303(g)(2)(i):Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:      a.  On or about 2/02/2017,  Electrical service panel  back of  the former Sunshine  Supermarket - electrical panel area:  Employees were using an electrical panel did that not have a dead front cover exposing live bus bars thus, exposing employees to electrical hazards.
